Please fill out the REQUEST FORM if you represent a health care facility, agency, or non-profit organization that is providing health care, food, shelter, and other essential services throughout the duration of the COVID-19 pandemic OR if you are an individual or family needing support due to self-quarantine, self-isolation or other extenuating circumstances.

We will do our best to match your request with the current donations and other offers of goods and services through business or community donations. Please let us know if you anticipate being out of supplies within a week and if your suppliers are unable to fill your request within that time period.   

 

Are you a State Agency in need of PPE or other supplies? 

​You need to submit your requests for PPE or other goods and services directly to the State Emergency Operations Center by emailing
seoc@ak-prepared.com.

Please remember:

• We cannot guarantee that you will receive all or ANY of your requested items. As stated above, we will do our best to match your request with the current business/community donations. We are continuing to monitor offers for goods and services as they are submitted. 
   

• We will fill resources based on life-safety priority.
   

• If your request is not fully filled, we will contact you to notify you of the amount we were able to allocate.
   

• Inventory will vary. Orders will be filled based on the supplies we have on hand.
